City doctors treat patient with two major cancers through rare surgery

Aurangabad: Using the Natural Orifice Transluminal Endoscopic Surgery (NOTES) technique, doctors of the city based MGM Hospital treated a patient with two major cancers in one surgery. Following endoscopic tests, it was confirmed that the patient had stage 1 cancers of oesophagus and stomach (gastric). The cancers in oesophagus and in stomach were of two cm and three cm in diameter, respectively.The histopathological tests confirmed the complete removal of the cancers. The surgery was performed three weeks ago and the patient has registered cent percent cure, Suryawanshi said. The endoscopic ultrasound clearly showed all the layers of the oesophagus, helping doctors understand the extent to which the tumour had grown.

The patient is doing fine and the theoretically there is no chance of the recurrence of the cancers, Pravin Suryawanshi, deputy dean at MGM Hospital, said.Earlier, such a patient might have been treated through open surgery, which requires long incisions and takes a patient a long time to recover from. There is also greater risk of infection and scars of stiches.Laparoscopic surgeries later became popular, and these were done with smaller incisions but nevertheless required a cut.Dean RB Bohra said, “In a significant recent form of surgery, NOTES is a procedure in which instruments are inserted through the natural openings in the body. There is no cut required to enter the body. As the passage is narrow, only one instrument can be used at a time. That requires a meticulous technique.”A 64-year-old male patient from Selu in Parbhani came to the hospital with complaints of difficulty in swallowing and acidity. Following endoscopic tests, it was confirmed that the patient had stage 1 cancers of oesophagus and stomach (gastric). The endoscopic ultrasound clearly showed all the layers of the oesophagus, helping doctors understand the extent to which the tumour had grown. As this was a case of early detection, the tumour had only grown up to the mucosa. This is why the doctors went for NOTES,” gastroenterologist Ashok Mohite said. The doctors conducted the surgical procedure through the mouth.The surgery lasted three and a half hours. The doctors first marked the the tumour, then raised the affected area by injecting saline, then cut out the tumour and removed it through the mouth. The cancers in oesophagus and in stomach were of two cm and three cm in diameter, respectively.The histopathological tests confirmed the complete removal of the cancers. The surgery was performed three weeks ago and the patient has registered cent percent cure, Suryawanshi said.
