State’s first valve-in-valve implantation performed in Nagpur

Nagpur: Noted cardiac surgeons Dr P K Deshpande and Swapnil Deshpande from city have claimed that a team of experts have successfully performed first transfemoral vein, trans-septal, mitral valve-in-valve implantation in the hybrid cardiac operation theatre of their hospital Dr K G Deshpande Memorial Centre. Earlier, she had gone open heart surgery in 2012 in our hospital and her valve was replaced successfully. So, Dr Vaijyanath was contacted. In December 2018, this valve got degenerated and patient started suffering from chest pain and other symptoms. After CT Scan, we concluded this valve needed to be replaced,” said Dr Swapnil.However, looking at the age and fragile condition of the patient, open heart surgery was not possible.

This is unique feat as it was the first such surgery in Maharashtra and only 16th in India.Cardiac Surgeon Dr Prashant Vaijyanath from Coimbatore and cardiologist Dr Gopalamurugan from Chennai who have expertise in this type of surgeries were in Nagpur on February 7 to help doctors from Dr K G Deshpande Memorial Centre.“The surgery was performed on a 68-year-old woman. Earlier, she had gone open heart surgery in 2012 in our hospital and her valve was replaced successfully. In December 2018, this valve got degenerated and patient started suffering from chest pain and other symptoms. After CT Scan, we concluded this valve needed to be replaced,” said Dr Swapnil.However, looking at the age and fragile condition of the patient, open heart surgery was not possible. So, Dr Vaijyanath was contacted. Interestingly, Dr Vaijyanath is GMC Nagpur alumni and he was intern under Dr P K Deshpande during his early years.“In this procedure, a tissue valve imported from Brazil was inserted through a vein like in any other angiography and then the valve was expanded on a balloon and placed properly on older valve. Patient has done remarkably recovery and will be discharged tomorrow,” said Dr Deshpande.According to him, the hybrid cardiac operation theatre in the hospital is the first requirement of such type of surgeries. It was set up way back in 2009. “I am happy that the cause for which the facility was set up is being served,” he said.Advantages:* Valve-in-valve implantation is safe and feasible option for elderly heart patients* Traditional treatment with redo surgical valve replacement can be dangerous in them* Transcatheter techniques adopting a valve-in-valve strategy can be a life saver for them* The technique is still under-adopted in India* India’s first transcatheter mitral valve implantation was performed in 2017How it’s done:* A catheter is inserted in the leg and guided to the heart* A replacement valve is inserted through the catheter* A balloon is expanded to press the valve in place on the heart
