AIIMS’ maiden replantation surgery fixes severed palm

Nagpur: In a remarkable achievement, the very first replantation surgery was successfully conducted at AIIMS Nagpur, giving a ray of hope for a 38-year-old patient who had suffered a devastating hand injury.The patients left hand was amputated in the middle of the palm in a compressor blast at Pusad, Yavatmal, on October 13, 2023. With prompt coordination between doctors at Pusad and AIIMS Nagpur, the patient reached the facility within the crucial time frame, setting the stage for an intricate surgical procedure. The 9-hour-long surgery was conducted at AIIMS by plastic surgeons Dr Dhananjay Nakade, Dr Al Iqyan Fidvi, and Dr Natasha, along with anaesthetists Dr Om Shubham Asai and Dr Avinash Prakash. The surgery required meticulous repair of various hand structures, from bones and blood vessels to tendons and nerves. Although the medical team successfully replanted the hand, the thumb tip could not be preserved due to the extent of injuries. The patients journey towards regaining function in the remaining four fingers and partial thumb will require prolonged physiotherapy, with optimism for improvement over time. Plastic surgeon Dr Dhananjay Nakade explained, Replantation surgeries are complex and require meticulous precision. We are pleased with the successful outcome. Such intricate procedures demand not only state-of-the-art surgical facilities but also seamless coordination in a dedicated medical team, from the initial emergency response to the patients eventual discharge. The key to success in such cases lies in the proper preservation of the severed limb or body part. Anaesthetist Dr Om Shubham Asai said, Preserving the amputated part is crucial. It should be placed in a plastic bag, then immersed in ice while avoiding direct contact with the ice. The chances of successful salvage increase significantly if the severed limb is brought to the hospital within 4 to 6 hours of the incident.Executive director Dr M Hanumantha Rao and officiating medical superintendent Dr Manish Shrigiriwar said that though such operations are commonly done at various other high end institutes, this is a first for AIIMS Nagpur.
